The office of Risk Management provides certificates of insurance to parties in the Peralta Community College District (CDD). The Risk Department collects certificates, reviews insurance requirements, and provides field trip waivers to students off-campus. External parties can reserve and pay for facility rentals through Facilitron.

Certificate of Insurance Request Form

A certificate of insurance is a document that demonstrates evidence of insurance policies in effect at the time of issuance. It is not an insurance policy, does not convey any rights to the certificate holder, and is often required to meet contractual obligations.

  1. Provide a copy of the Contract/Agreement/Resolution/Memorandum of Understanding (MOU)/Independent Contractor Contract (ICC).
  2. Name of the organization the certificate should be made out to.
  3. Complete address, including zip code.
  4. “Attention to” name.
  5. If an additional insured or loss payee is required, please allow up to a week for processing.
  6. Forward the information to riskmanagement@peralta.edu
Field Trip Waiver

Field Trip Waiver

Field trips involve activities in environments that may present risks not typically encountered in the classroom. To help ensure the safety of all participants, faculty, and staff, organizing field trips requires following district policies and guidelines to plan appropriately for potential risks.

As part of this process, a field trip waiver and request form must be completed and approved prior to student participation in any off-site classroom activity.

How to complete the field trip waiver:

  1. Complete the fillable form electronically (see above)
  2. Obtain AP, VP, or Dean's approval
  3. Submit the approved field trip form to risk management prior to the scheduled field trip
  4. Retain a copy of the approved form for your records
  5. Risk management will review the submission and maintain official records once approval is completed

Facilitron helps schools manage and lease their athletic fields, classrooms, and more, providing an easy way for communities to reserve and pay for a variety of facility rentals.
