Overview

General liability claims usually involve property damage, theft, vehicle accidents, and medical incidents. These claims are for third parties, such as students and visitors.

  1. It is essential that claims be reported within 24 hours to risk management, regardless of the severity of the claim.
  2. For all injuries, including bodily injuries, please complete the “Accident and Claim form” listed below.
  3. Liability Claims are investigated and managed by outside claims administrator, Keenan. If you are contacted by an adjustor from Keenan, please respond promptly with any request for information.

Accident Reporting and Claims

Claims coverage and protection for accidents. Slips, falls, and other incidents can cause injury to yourself and others. Claims also include destruction of property.

 

Peralta Property Damage or Theft

  1. If there has been damage/loss to a building or property, or theft in your department, contact the department supervisor.
  2. Contact the Peralta Department of Community Safety and the police department to file a report as soon as possible.

  3. Take photos
  4. Generate a list of property damaged or stolen additionally submit the itemized costs.
  5. Submit a Peralta Public Safety Report, Police Report, photos, and the list of equipment and damages to the office of risk management at Riskmanagment@peralta.edu
Reporting a Medical Incident

Reporting and documenting an accident, injury, or medical emergency on Peralta Community College District property. Parties engaged in college business, while on college trips, and attending college-sponsored events. Parties include students, visitors, and volunteers.

Medical injuries such as slip and fall accidents, sports injuries, general bodily harm such as such as broken bones, sprains, strains, cuts, and lacerations are a few examples of medical injuries.

How to report a medical incident:

  1. Please contact 911 immediately for life-threatening injuries.
  2. Report your injury immediately to your supervisor.
  3. Complete the Medical Incident Report and submit it to Risk Management as soon as possible.
  4. Risk management will contact you as soon as possible.

Peralta Vehicle Accident

Peralta Vehicle Accident

Submit a Vehicle Accident if you have been involved in an accident with a Peralta vehicle or if you are a driver of a Peralta Vehicle involved in an automobile accident.

If you are involved in a Peralta Vehicle accident, please follow the next steps:

  1. Injury: Call local police and or 911 for life-threatening injuries
  2. Non-injury: Exchange insurance information and file a police report online
  3. Takes photos of the accident
  4. Fill out the Vehicle Accident Report Form
  5. Forward the Vehicle Accident Report, photos, and police report to the office of Risk Management for processing: Riskmanagment@peralta.edu
