Peralta Reimbursement Programs
If you are enrolled in a SISC medical plan (Kaiser, Kaiser Senior Advantage, or Anthem), medical and prescription reimbursements are processed by Navia.
If you are enrolled in the District’s Anthem MAPD plan or Traditional Medicare with the United American Supplemental plan, reimbursements are processed by the District.

If you are enrolled in Retiree First’s Anthem Medicare Advantage Prescription Drug Plan or Traditional Medicare w/ United American Supplemental Plan, you must submit reimbursement requests to the Office of Human Resources for review and approval. Prior to processing your claim, you will be required to submit a vendor application and W-9 form to the Office of Human Resources (if you have not been issued a vendor number as a retiree). Upon receipt of your vendor number, your claim will be processed within 30 days.

Changes to Medical plans can only be made during Open Enrollment, or if you experience a Qualifying Life Event. If you are enrolled in a SISC plan (Kaiser, Kaiser Senior Advantage, or Anthem), please contact the Benefits Office for the applicable forms. Forms may be emailed or mailed upon request.
